New Patient Registration

Each practice has a catchment area within which they can accept new patients.

Step 1 - Check you are within the practice's catchment area

If you are unsure please contact the surgery and we will check for you

North: River Alt between Hall Lane and Mairscough Lane

South: Between Sefton Lane and Hall Lane Maghull

East: Railway line between Moss Lane Lydiate and Park Lane Maghull

West: Bells Lane Lydiate

Step 2 - Pick up a New Patient Registration Form from Reception

Step 3 - Complete Registration Form

The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the to quickly trace your medical records and send them to your new surgery. The New patient questionnaire will ask you for some basic, lifestyle and allergy questions, so we can provide the best service possible from the start. If you are on repeat medication please ensure you have obtained 1 months supply prior to registration.

The new patient registration form is a legal document, and will require the following documentation as proof of residency
