Firearms Licence Applications & Renewals

Important Information for Patients

The Partners at High Pastures Surgery have taken the decision not to complete medical reports, health forms, or provide medical assessments in support of firearm or shotgun licence applications.

This includes:

  • Firearm and shotgun certificate medical forms
  • Medical reports requested by firearms licensing departments
  • Ongoing firearm licence medical reviews and proforma completion

Our clinicians' time is dedicated to providing NHS medical care and services for our patients. As a result, patients who require medical information for firearms licensing purposes will need to seek assistance from an alternative provider.

Alternative Provider Guidance

Patients may wish to:

  • Contact a private GP service that offers firearms licensing medical assessments.
  • Search online for a "firearms medical report service" or "shotgun/firearm licence medical assessment" in their local area.
  • Check with the relevant police firearms licensing department, who may be able to provide information about the medical evidence required for an application.

Please note that any arrangements made with an alternative provider are the responsibility of the applicant, and any fees charged will be payable directly to that provider.
