What it means for our patients

Most patients do not need to do anything. If your details are correct and you still live within our practice area, no action is required.

Why is this happening?

The NHS is reviewing patient records across England to make sure information is accurate and up to date. This helps identify patients who have moved house, moved outside our practice area, have incorrect contact details, or may no longer be registered with the most appropriate GP practice.

How could this affect me?

You may be contacted to confirm your address, update your telephone number or email address, check you still livewithin our practice area, or confirm your registration details.

What should I do?

Please let us know if you have recently moved home, changed your telephone number, changed your email address, or moved into or out of the Maghull or Lydiate area.

Why does it matter?

Accurate patient records help us contact patients quickly, send important health information, improve appointment access, plan services, ensure NHS resources are directed where needed most, and provide safe, high-quality care.
